Rural Alabama Prevention Center (RAPC) receives funding from the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA), and the Delta States Rural Development Network Grant Program for the implementation of the South West Alabama Health Improvement Initiative (SWAHII).
The goals of SWAHII are:
a) reduce the prevalence of chronic disease in South West Alabama, and
b) reduce health care utilization and cost of persons with or at-risk of developing chronic diseases.
The objectives of SWAHII are:
a) reduce frequent use of the emergency room,
b) prevent unnecessary hospitalization,
c) reduce repeat hospital admission, and
d) support individuals in making lifestyle changes to prevent or reduce the impact of chronic disease and its associated risk factors through prevention and management strategies.
